Citizens Battery Backup Waiver

NO THANKS, I decline the installation of an additional device in my home for an 8-hour battery backup for my Citizens Telephone (VoIP) Service. I understand by signing below that my Citizens telephone service will not be operational during a power outage unless the Citizens equipment is powered by a generator or some other device.
